2. Questionnaires / Preliminary Information

What does a spermiogram show – and what doesn’t?

A spermiogram is a snapshot. It helps to assess sperm quality and further clarify possible causes. At the same time, individual stocks should never be valued in isolation. Stress, infections, fever, lack of sleep or alcohol can temporarily affect results.

Preparation – so that the values are meaningful

For good comparability, many centers recommend a waiting period (no ejaculation) of 2 to 5 days, ideally often 3 to 5 days.
Also helpful:

  • 24–48 hours in advance, no alcohol, no sauna sessions/heat exposure

  • in case of acute infections/fever, postpone appointment if necessary (values may be falsified)

  • Please state medications/dietary supplements openly

How “fresh” does the sample have to be?

For a reliable assessment, the sample should be analyzed promptly . For this reason, delivery is usually carried out directly in the centre, so that mobility and other parameters can be assessed as unbiased as possible.

What options are there for conspicuous values?

An abnormal spermiogram does not automatically mean that pregnancy is ruled out. There are often sensible next steps – from a new check-up to targeted treatment options.

Depending on the findings, the following can be considered, among others:

  • Control spermiogram (because values can fluctuate)

  • Supplementary diagnostics (e.g. hormones, ultrasound, genetic clarification if necessary)

  • Recommendations on lifestyle factors (weight, nicotine/alcohol, sleep, heat)

  • Fertility treatments: e.g. IVF or ICSI (depending on the couple’s situation)

Can I have a “second spermiogram”?
Yes. Since a spermiogram is a snapshot and values can fluctuate, a check-up appointment is often useful – especially if the first result was borderline or abnormal. Please bring any previous findings with you.

What should I avoid before handing it in?
The most important thing is the appropriate waiting period (usually 2–5 days, often ideally 3–5). In addition, alcohol, intense heat (sauna/hot baths), lack of sleep and acute infections can impair the informative value.

Why should the sample be delivered directly to the center?
Because certain values – especially mobility – become more difficult to assess over time. By submitting the sample on site, the sample can be processed quickly.

What are common causes of constrained values?
Possible factors include genetic influences, previous illnesses, hormonal changes, varicocele (varicose veins on the testicles), infections, heat exposure or lifestyle factors.

What can worsen sperm quality?
Nicotine, high alcohol consumption, obesity, chronic stress, lack of sleep and regular heat exposure (sauna/hot baths/tight clothing) often play a role.

Can you become a father despite an abnormal spermiogram?
Yes. Many couples become pregnant despite abnormal values – sometimes naturally, sometimes with support. The decisive factor is the classification of the findings and the appropriate procedure as a couple.
